You need to specify the final title you intend to publish under this exemption program. Without the final title, you will not be qualified.

Please describe your Derivative Work you intend to publish.
What if I'm making a free game and I wanted to see if I can implement it in a game engine? What should I post in those fields?

I'm not sure what you meant by "implement it in a game engine", but make sure it does not breach the SDK License Agreement.
Neither modification/reverse-engineering of SDK nor developing Extendable Application is allowed.
